If your child has a congenital heart defect, his care and needs will change as he enters adulthood. This transition period must be carefully coordinated, with pediatric and other healthcare providers working together. In partnership with Emory’s Heart and Vascular Center, our Children’s Sibley Heart Center is working to help bridge gaps in cardiac care for children and adults with congenital heart defects.

The Congenital Heart Center of Georgia is one of the largest transition of care programs in the nation and the only one in Georgia. The comprehensive center works together to coordinate a seamless transition from pediatric to adult care for thousands of patients. In doing so, we help ensure that all patients—children and adults—receive the best possible care from dedicated professionals specially trained to monitor their condition.
