Get Real About Diabetes (GRAD) Program
The GRAD program is designed to improve diabetes management by helping teens become more confident with their diagnosis. It is open to patients with Type 1 and Type 2 diabetes. The program is on an individual basis with visits occurring every 4 to 6 weeks.
- Visits replace routine diabetes appointments as patients involved in the program will see a nurse practitioner and social worker each time.
- Due to the organization of visits, the program is available only for patients receiving care within the Children's Physician Group. If you are interested in entering the GRAD program, talk to our staff at your next appointment.
