Medically Complex patients are broadly defined as those with three or more specialists and conditions for which our Complex Care program has been designed to support, with three main objectives: collectively improving the health of the Medically Complex Population, improving the patient and family’s experience of care, and reducing the overall cost of care - in ways that haven’t been provided in Georgia before. Your support will directly impact the dedicated medical management team's programs and growth that will promote continuity of care and ultimately improved outcomes and prevention, through early identification and intervention of not only medical but also psychosocial disorders, by providing essential services for our most vulnerable and underserved population.
