For Parents/Caregivers 

  • Children's Rehabilitation Associates
    If you have been referred to the Physiatry or Concussion Clinic, call 404-785-2490 to make an appointment.
  • Comprehensive Inpatient Rehabilitation Unit (CIRU)
    Call 404-785-2274, 8 a.m. to 6 p.m., Monday through Friday.
  • Day Rehabilitation Program
    If your pediatrician has referred you to Children’s Day Rehabilitation Program, call 404-785-3300.
  • Outpatient Rehabilitation Program
    If you have been instructed by your pediatrician to visit Children’s Outpatient Rehabilitation Program, have your child's doctor fax the prescription to 404-785-7113. Make sure the following information is included on the order: Patient name, DOB, diagnosis, ICD9 code and treatment needed, along with doctor's name and license number, date, phone and fax numbers. Contact our central business office at 404-785-7100 to schedule an appointment. You will need to know the patient’s medical diagnosis and treatment needed to schedule an appointment or use our online preregistration form.


All Children’s Healthcare of Atlanta Rehabilitation Services require a prescription from a Georgia-licensed physician. Please speak with your child’s healthcare provider regarding services.
