How to Refer a Patient to Children's
To make a referral in your area, please use the following guidelines.
- Children's Rehabilitation Associates
To refer a patient to the Physiatry Clinic, please call 404-785-3800.
- Comprehensive Inpatient Rehabilitation Unit (CIRU)
To refer a patient to our CIRU, you may either contact one of our evaluators or fill out our referral form for professionals.
- Day Rehabilitation Program
To refer a patient to our Day Rehabilitation Program, please call 404-785-3300.
Please have the following information available when making a CIRU or Day Rehabilitation referral:

Outpatient Rehabilitation Program
To refer a patient for outpatient rehabilitation at one of our eight locations, please fax a physician prescription to 404-785-7113. Please include the following information on the prescription: Patient name, gender, DOB, diagnosis, ICD9 and treatment needed along with physician name, phone and fax numbers. Please advise parent to call 404-785-7100 to make an appointment. Parent MUST know medical diagnosis and services needed to schedule appointment or use our online pre-registration form.
