Request Copies of Your Child's Medical Records

We are happy to help you get medical records. Please note the steps below. See our frequently asked questions for more information.

Step 1

Before your child's medical records can be released, the legal guardian must complete, date and sign a release of information authorization form. (En Español)

If the child is over the age of 18, he/she must request the information personally. The only exceptions are for patients who are 18 or older or patients who are mentally or physically unable to sign (we require documentation for this situation).

Step 2

Mail the completed release form to our medical records department, which is located at Scottish Rite hospital.

Step 3

We will mail you an invoice based on the number of pages being released. 

Once we receive your payment, we release the records. You may request to pick up the records at the nearest Children’s hospital location or have the records mailed to you.

Pick Up Records

We are not able to fax records. You may make an appointment to pick up copies in person. Otherwise, records will be mailed.
