Sports Medicine Program Quick Reference Referral Information

The patient’s primary care physician and/or specialist needs to write a prescription for Physical Therapy/Sports Medicine--referrals are available for patients age 7 to 21. The prescription should identify the following:

  • Diagnosis
  • Request from physician to evaluate and treat (or specific protocol if available)
  • Length of treatment
    • Example: Two times a week for four weeks
    • Physician signature

    After the patient is referred by his/her primary care physician and/or specialist, the patient/parent of patient should call one of our below locations to set up an appointment.
