SurvivorLink is an information technology network currently in development that will serve as a crucial tool for helping ensure childhood cancer survivors in Georgia receive the recommended survivor care, and as a result, optimal health and quality of life.
The electronic system will include:
- Survivor Healthcare Plan
- Individualized risk profile
- Personal surveillance plan
- Educational materials to improve awareness
- National guidelines for survivorship care
- Other clinical information needed to provide long-term care
Whether a patient resides in rural Georgia or metro Atlanta, SurvivorLink will electronically facilitate communication and share information among the:
- Survivor
- Survivor team
- Primary care physician
- Any subspecialist
