A Survivor Healthcare Plan is a longterm, follow-up plan based on national guidelines.
In preparation for a patient's first visit to a cancer survivor program, a Survivor Healthcare Plan should be developed. This "roadmap to survivorship" is tailored to a survivor's individual treatment and includes:
- Medical summary of cancer diagnosis and treatment
- Individualized risk profile
- Personalized surveillance plan
